Inspector observed light gap underneath rear door. Corrective Action: outer openings of a food service establishment shall be protected against the entry of insects and rodents by solid, self-closing, tight-fitting doors.

Inspector observed mops placed in mop bucket. Corrective Action: After use, mops shall be placed in a position that allows them to air-dry without soiling walls, equipment, or supplies.

Observed prepped cheese stuffed crust pizza dough out at room temperature measured 64F. COS - discarded and provided PIC with TPHC form to submit for review. Advised PIC to only make cheese stuffed crust to order until they submit TPHC form for health dept review AND until TPHC is approved by local health authority; and cold-hold cheese sticks at 41F or below.

Observed 5 prepped stuff crust pizza dough with the cheese and or cheese and pepperoni in the crust out at room temperature at 61F. COS- dough was discarded. Advised PIC to only make crust to order OR submit TPHC for health dept review. TPHC forms were provided to the CFSM during the inspection.

Most recent copy of health inspection was not posted. Advised PIC to post most recent copy of health inspection report. 511-6-1.02(1)(d) - Displaying of the Inspection Report (C)

1. The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away.
