Observed no plan or procedures for key drop deliveries made during non-operating hours.
CA: The PIC must ensure the facility has a written procedure and plan that are implemented and maintained where required for key drop deliveries.
COS: Written plans and procedures along with corrective actions will be maintained and recorded and readily available for review by health dept.-Health Dept. will re-check records
Check 2/3/26 CFSM Tyler Buice has decided not to do Key Drop Deliveries and deliveries will be at a time when employees are there to receive the food.

observed multiple personal drinks stored on prep tables and with lid and straw. CA: Personal drinks must be in single use cups with lid and straw and be stored away from food prep surfaces or restricted areas of kitchen. COS: pic discarded personal drinks

observed milk dated best by December 22/2025, half n half dated best by 1/23/26 and honey mustard dated best by 1/16/26. CA: Foods must be discarded by the expiration date. COS: pic discarded food
12D - washing fruits and vegetables
Regulation: 511-6-1.04(4)(g) - washing fruits & vegetables (pf)
Observed raw/undercooked animal foods being prepared/stored in fruit and vegetable sink. CA: Sink used for washing fruits and vegetables should be designated and not used for preparation or storage of raw animal foods. COS: pic removed raw chicken and cleaned and sanitized sink.
14B - utensils, equipment and linens: properly stored, dried, handled

Observed knives stored in crevices between equipment and wall. CA: Utensils must be stored in a clean location designed for storage. COS: pic removed knives to be cleaned and sanitized.
