Observed chipped and peeling paint on surface of bathtubs in rooms 204, 401, and 412. Corrective Action: Discussed with PIC. Tubs must be properly repaired and repainted by a licensed professional. This will be rechecked

Observed black buildup on bathtub in room 219. Corrective Action: PIC was notified. Housekeepers were assigned to properly clean and sanitize bathtub onsite

Observed buildup and debris in overhead bathroom vent covers of all rooms inspected. Corrective Action: PIC was notified. Maintenance will be assigned to clean all bathroom vent fans. This will be rechecked

Observed trash and food debris on floors behind beds in rooms 204, 219, 305, 401, and 412. Corrective Action: PIC was notified. Housekeeping was assigned to clean floors behind all beds onsite

Observed black build-up on interior surfaces of guest ice machine on 3rd floor. Corrective Action: Owner was notified. Discussed regular cleaning and sanitizing of ice machines to prevent build-up. Employee was designated to clean and sanitize ice machine onsite

Discussed regularly cleaning and sanitizing ice machines to prevent build-up. PIC will designate employee to clean and sanitize ice machine. Maintenance will service machine and check drain line for possible leak

Observed dust-build up on air vent filters in rooms 205, 223, 305, 319, and 404. Air vent was observed to be missing in room 319. Corrective Action: PIC was notified. Housekeeping will clean and/or have maintenance replace air filter

Observed food and debris build-up on inner surfaces of microwaves in guest rooms 121, 205, 223, 404 and 421.
Corrective Action: PIC was notified. PIC will re-train housekeeping and have all microwaves properly cleaned

Observed ceiling air vent in clean linen storage of hotel laundry with significant dust build-up. Dust build-up was also noted on air intake vents in wash area. Corrective Action: Discussed cleaning frequency of air vents in laundry room with PIC. Storage areas for storing clean linens and laundry must be at least 6-inches off the floor and be free of pests, vermin, dust and moisture. Maintenance will clean all vents in the laundry room and increase frequency

Observed phone in room 420 with broken connection. CA: PIC will have phone repaired

Observed faucet handle in room 420 unable to produce flow of warm water when handle is turned to hot water. CA: PIC will have sink handle repaired and will notify Health Department upon completion of servicing

Observed mechanical ventilation fan in guest room 424 not turning on.

Corrective Action: PIC was notified. Maintenance will repair. Room 424 should not be rented until bathroom has proper ventilation

Observed dust build-up in the air filters for rooms 424 and 425.

Corrective Action: PIC notified. Maintenance will clean and/or replace the air filters

Observed microwave in room 404 with food build-up. Corrective Action: Discussed cleaning procedure and frequency with PIC. PIC will designate housekeeper to clean microwave

Observed broken face plate, grate and filters on air conditioning unit in room 404. Corrective Action: Discussed with PIC. The face plate of the unit will be replaced by maintenance

Observed ice machine on 1st and 3rd floor with black build up in ice chute. The ice maker is currently out of service on the 1st floor. Corrective Action: Notified PIC and discussed cleaning procedure and frequency. The ice maker on the 3rd floor will be put out of service and properly cleaned and sanitized by hotel maintenance. The Health Department will recheck this before it available for guest use
