Observed no signed employee health policy agreements for several employees. CA: Food employees shall be informed in a verifiable manner of their responsibility to report symptoms and illnesses related to diseases that are transmissible through food. COS: No correction on site can be observed. Management must inform employees in a verifiable manner of their responsibility to report symptoms and illnesses related to diseases that are transmissible through food.

Observed in-use wiping cloths stored in chemical sanitizing solution not at required concentration. CA: Cloths in-use for wiping counters and other equipment surfaces shall be held between uses in a chemical sanitizer solution at the required concentration. COS: Sanitizer buckets were remade at the correct concentration.

Observed evidence of water leak on the floor possibly coming from water heater. CA: Equipment shall be maintained in a state of repair. COS: No correction on site can be observed. Facility must have water leak repaired. This will be rechecked by health dept.

Observed ice condensation build-up in walk-in freezer. No contamination was observed as trays were being used to prevent any drippage on food boxes. CA: Equipment shall be maintained in a state of repair. COS: No correction on site can be observed. Facility must have walk-in freezer serviced/repaired. This will be recheck by the health dept.

Observed food handlers preparing food with jewelry on arms. CA: Discussed jewelry policy with pic. COS: all employees removed bracelets and washed hands before working with food.

During routine inspection, observed food service employee arrive to start their shift. Upon entering the kitchen, food service employee did not wash hands. Employee was observed putting away personal items and putting on hair restraint. Employee was then seen wearing gloves and stated they did not wash hands before putting on gloves to begin working with food.

Corrective Action: PIC was notified and instructed food service employee to remove gloves and wash hands. Discussed when to wash requirements with PIC. PIC will train all food service employees on washing hands.

Observed residue buildup on multiple soda machines around soda nozzles. No residue buildup was detected on soda nozzles or ice dispenser. CA: Discussed procedures for cleaning soda machine with PIC. PIC will have soda machines cleaned and increase overall cleaning frequency.

Observed employee with watch on while prepping food. CA: PIC removed watch.

Observed employee with excessively long acrylic fingernails without gloves in drive through area. CA: PIC will educate employees concerning jewelry and fingernail length in food prep areas.
