Observed no signed employee health policy agreements for several employees. CA: Food employees shall be informed in a verifiable manner of their responsibility to report symptoms and illnesses related to diseases that are transmissible through food. COS: No correction on site can be observed. Management must inform employees in a verifiable manner of their responsibility to report symptoms and illnesses related to diseases that are transmissible through food.
12C - wiping cloths: properly used and stored
Regulation: 511-6-1.04(4)(m) - wiping cloths, use limitation (c)
Observed in-use wiping cloths stored in chemical sanitizing solution not at required concentration. CA: Cloths in-use for wiping counters and other equipment surfaces shall be held between uses in a chemical sanitizer solution at the required concentration. COS: Sanitizer buckets were remade at the correct concentration.

Regulation: 511-6-1.05(6)(a) - good repair & proper adjustment (c)
Observed evidence of water leak on the floor possibly coming from water heater. CA: Equipment shall be maintained in a state of repair. COS: No correction on site can be observed. Facility must have water leak repaired. This will be rechecked by health dept.
15A - food and nonfood-contact surfaces cleanable, properly designed, constructed, and used
Observed ice condensation build-up in walk-in freezer. No contamination was observed as trays were being used to prevent any drippage on food boxes. CA: Equipment shall be maintained in a state of repair. COS: No correction on site can be observed. Facility must have walk-in freezer serviced/repaired. This will be recheck by the health dept.
