Observed grey water discharge on ground surface behind facility. Grey water discharge is due to damaged and/or disconnected plumbing behind facility from main drain of primary laundry facility. Corrective Action: Owner was notified. Licensed plumber must be contacted for immediate repairs to main laundry discharge line. This grey water line must be properly plumbed into main sewer line for entire facility as required. Documentation is required. Health Dept will recheck

Observed damaged exterior mounting box for storing fire extinguisher. Due to damaged box, fire extinguisher was not stored for access to the public in case of an emergency. Owner is keeping fire extinguisher in her office. Corrective Action: Owner was notified that fire extinguisher must be stored for access to guests in the event of an emergency. Storage box must be repaired or replaced asap. This will be rechecked

Observed smoke detector not working in main laundry facility. Corrective Action: Owner was notified. Smoke detector will be repaired or replaced, then rechecked

Observed nonfunctional smoke detector in room 106. Corrective Action: Discussed ensuring smoke detectors are available/ functional in all rooms. PIC replaced battery in smoke detector

Observed daylight coming through cracks in exterior doors of rooms 101 and 106 when the doors are closed. Corrective Action: Discussed replacing/ adding weather stripping to all exterior doors with PIC. PIC will order and replace weather stripping to ensure all exterior doors are properly sealed

Observed writing left by previous guests in room 101 and 106. Observed writing on bathroom wall in room 101. Observed writing on bathroom mirror, on headboard next to the bed, and above the door in room 106. Corrective Action: Discussed keeping walls cleaned with PIC. PIC will remove/ cover the writing prior to renting both rooms

Observed burn holes and stains in bedspread and sheets in guest room 101. Observed burn holes in bedspread, sheets and mattress pad in guest room 106. Corrective Action: Discussed keeping linens and bed coverings clean and in good condition with PIC. If linens are damaged or burned, they should be discarded. PIC will replace linen and discuss with housekeeping

Observed most recent inspection not posted at time of inspection. Corrective Action: Discussed posting requirements with PIC. The most recent inspection report must be prominently displayed in public view at all times

Observed missing outlet cover behind bed in room 106. Corrective Action: PIC was notified. PIC will replace missing outlet cover before renting the room

Observed broken dresser handles in room 101. Observed trash in drawers of room 101 and trash under dresser in room 102. Corrective Action: Owner was notified. Housekeeping was assigned to clean these areas. Handles will be repaired or replaced. This will be rechecked

Observed broken tub faucets not operating properly in room 102. Corrective Action: Owner was notified. This room will be placed out of service. Proper repairs will be conducted and rechecked
