bserved several large stains u on the following items: Mattress cover on bed in room 103, mattress cover and mattress pad on bed near window and on mattress cover on bed near bathroom in room 104 ; Room 228- mattress cover and mattress pad on bed near bathroom and mattress cover on bed near window in room 205. ca:Beds, mattresses, mattress pads, springs, frames, pillows, and bed coverings clean; vermin free and in good condition; bed linen properly supplied, changed and laundered

rooms inspected lacked the antislip condition. this was observed in ROOM, 103, ROOM: 104 AND ROOM 228 CA: Anti-slip tubs/showers, slip strips, mats or appliqué products provided and in good condition

Observed several large stains under black light on the following items: Mattress cover on bed in room 101, mattress cover and mattress pad on bed near window and on mattress cover on bed near bathroom in room 121; mattress cover in room 122; Flat sheet, mattress pad and mattress cover on bed in room 227; mattress cover and mattress pad on bed near bathroom and mattress cover on bed near window in room 205. CA: Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be maintained clean

Observed toilet escutcheon separated from wall or missing around toilet pipe in the bathroom of all rooms inspected. CA: Toilet facilities shall be maintained in good repair

Observed the following stored without labels of its contents: 2 bottles stored on bottom shelf in housekeeping laundry, 2 bottles on housekeeping cart and 5 bottles on yellow shopping cart on ground floor; 3 bottles on housekeeping cart of 2nd floor. CA: Any spray bottle used in cleaning a tourist room or stored on the housekeeping cart shall be labeled with its contents
