OBSERVED SEVERAL FOODS STORED IN REACH IN COOLER (NEAR DEEP FREEZER) IN THE MAIN KITCHEN UNCOVERED AND NOT PROTECTED FROM CONATMINATION. (I.E. RAW SHRIMP, CORN W/SAUSAGE AND POTATOES, RAW TILAPIA, RAW WHITING, RAW CALAMARI, RAW OYSTERS, RAW SCALLOPS, AND RAW CATFISH.) CA: (iv) Except as specified under paragraph 2. of this subsection and when cooling as specified in DPH Rule 511-6-1-.04(6)(e)2.(ii), storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination;

OBSERVED UNCLEAN UTENSILS STORED AS CLEAN IN THE WAREWASHING AREA (I.E. SPOONS, GLASS CUPS, FOOD GRADE PLASTIC CUPS, FOOD GRADE PLASTIC STRAINERS, TONGS, AND KNIVES). OBSERVED DARK DEBRIS ON CHUTE OF ICE MACHINE. CA: Equipment, Food-Contact Surfaces, and Utensils.
- Equipment food-contact surfaces and utensils shall be clean to sight and touch. Pf.
6-2 - proper date marking and disposition

OBSERVED NO DATE MARKING SYSTEM FOR FOODS BEING HELD OVER 24HRS THROUGHOUT ENTIRE FACILITY (I.E. PAR COOKED CHICKEN, FRIED RICE, SLICED TOMATOES, CUT LETTUCE, ETC.). CA: Refrigerated, ready-to-eat, time/temperature control for safety food prepared and held in a food establishment for more than 24 hours shall be clearly marked to indicate the date or day by which the food shall be consumed on the premises, sold, or discarded, when held at a temperature of 41°F (5°C) or below for a maximum of 7 days. OBSERVED 8 SQUEEZE BOTTLES NOT EASILY INDENTIFIABLE UNLABELED IN THE MAIN KITCHEN PREP AREA. CA: Food Storage Containers, Identified with Common Name of Food. Except for containers holding food that can be readily and unmistakably recognized, such as dry pasta, working containers holding food or food ingredients that are removed from their original packages for use in the food establishment, such as cooking oils, flour, herbs, potato flakes, salt, spices, and sugar shall be clearly and legibly identified, in English, with the common name of the food.

OBSERVED FROZEN RAW SHRIMP BEING THAWED IN RUNNING WATER ABOVE 70 DEGREES F. CA: Except as specified in paragraph 4 of this subsection, time/temperature control for safety food shall be thawed:
-
Under refrigeration that maintains the food temperature at 41°F (5°C) or less; PF or
-
Completely submerged under running water:
(i) At a water temperature of 70°F (21°C) or below, PF
(ii) With sufficient water velocity to agitate and float off loose particles in an overflow PF, and
(iii) For a period of time that does not allow thawed portions of ready-to-eat food to rise above 41°F (5°C), or
(iv) For a period of time that does not allow thawed portions of a raw animal food requiring cooking to be above 41°F (5°C), for more than 4 hours including:
(I) The time the food is exposed to the running water and the time needed for preparation for cooking PF, or
(II) The time it takes under refrigeration to lower the food temperature to 41°F (5°C) PF;
- As part of a cooking process if the food that is frozen is:
(i) Cooked as specified under subsections (5)(a)1 or 2 or (5)(b) or 5(e) of this Rule PF, or
(ii) Thawed in a microwave oven and immediately transferred to conventional cooking equipment, with no interruption in the process PF; or
-
Using any procedure if a portion of frozen ready-to-eat food is thawed and prepared for immediate service in response to an individual consumer's order.

Reduced oxygen packaged fish that bears a label indicating that it is to be kept frozen until time of use shall be removed from the reduced oxygen environment:
(i) Prior to its thawing under refrigeration that maintains the food temperature at 41°F (5°C) or less; or
(ii) Prior to, or immediately upon completion of its thawing, using procedures to completely submerge in running water as specified in paragraph 2 of this subsection.
14A - in-use utensils: properly stored
Regulation: 511-6-1.04(4)(k) - in-use utensils, between-use storage (c)
OBSERVED HANDLES OF SCOOPS AND BOWLS BEING STORED DIRECTY INTO FOOD PRODUCTS IN THE MAIN KITCHEN AREA (I.E. RICE, SUGAR, SALT, UMAMI SEASONING, BROWN SUGAR, AND PAPRIKA). OBSERVED HANDLES OF SCOOPS STORED DIRECTLY INTO ICE IN ICE MACHINE IN THE FRONT FOOD SERVICE AREA. CA: Storage of In-Use Utensils. During pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored:
1.Except as specified under paragraph 2 of this subsection, in the food with their handles above the top of the food and the container;
2.In food that is not time/temperature control for safety food with their handles above the top of the food within containers or equipment that can be closed, such as bins of sugar, flour, or cinnamon;
3.On a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized at a frequency specified under DPH Rule 511-6-1-.05(7)(b) and (8)(a);
4.In running water of sufficient velocity to flush particulates to the drain, if used with moist food such as ice cream or mashed potatoes;

In a clean, protected location if the utensils, such as ice scoops, are used only with a food that is not time/temperature control for safety food; or
-
In a container of water if the water is maintained at a temperature of at least 135°F (57°C) and the container is cleaned at a frequency specified under DPH Rule 511-6-1-.05(7)(b)3(vi).
