Observed no allergy disclosure in the facility. CA: The permit holder shall notify consumers by written notification of the presence of major food allergens as an ingredient in unpackaged food items that are served or sold to the consumer. PIC will create an allergy disclosure with the major allergens that are used in the facility.

Observed employee beverages stored in reach in cooler among facility food product. CA: Areas designated for employees to eat, drink, use tobacco products and electronic devices shall be located so that food, equipment, linens, and single-service and single-use articles are protected from contamination. PIC will have a designated area for employees to store food and beverages.

Observed leaking pipe underneath the 3-compartment sink. C/A: A plumbing system shall be repaired according to law; P and maintained in good repair. PIC will have the plumbing repaired.

Observed pan liners stored uncovered. C/A: Except as specified in paragraph 4 of this subsection, cleaned equipment and utensils, laundered linens, and single-service and single-use articles shall be stored: Where they are not exposed to splash, dust, or other contamination. PIC will store pan liners covered in a location that is not exposed to splash, dust or contamination.

Observed a temporary employee working in the kitchen that has not signed an employee health policy. CA: Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with the Chapter, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food. PIC will have conditional employees sign an employee health policy.

Observed no hot water at the handwashing sink in the restroom. CA: Hot water generation and distribution systems shall be sufficient to meet the peak hot water demands throughout the food service establishment. PIC will have the sink repaired to have hot water.

Observed fume hood with an accumulation of dust. CA: Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. PIC will have the fume hood cleaned.

Observed missing floor tiles in the main kitchen area under the RIC. CA: Except as specified under subsection (2)(d) of this Rule and except for antislip floor coverings or applications that may be used for safety reasons, floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. PIC will have the floor tiles repaired.
