Observed the sheet on the bed in room 115 not clean to sight/torn. CA: Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. PIC will have sheet replaced with one that is not torn and that is clean

Observed the sheet on the left bed not clean to sight and stained pillows in room 137. CA: Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly. PIC will have sheet changed out with a clean one and pillows cleaned or replaced with ones without stains

Observed two stained pillows in room 139. CA: Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. Observed the toilet in room 137 not clean to sight/clogged. CA: Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Walls, floors, and ceilings shall be constructed of easily cleanable nonabsorbent materials and shall be kept clean and in good repair. While being used by tourists, every surface of a bathtub, shower, shower enclosure, toilet and lavatory, which may come in contact with a person’s body, must be sanitized each day, unless the guest has declined regular tourist room services. If a tourist declines regular tourist room services, the tourist accommodation facility must ensure that these surfaces are cleaned and sanitized at least once per week and between tourists. PIC will have toilet cleaned/unclogged

Observed the ceiling in the bathroom of room 115 cracked/falling down. CA: Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean. PIC will have ceiling repaired

Observed stains on mattress pad in room 139. CA: Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. PIC changed out mattress pad

Observed stained pillowcase in room 120. CA: Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. PIC changed the pillowcase

The premises shall be kept in such condition as to prevent the attraction, harborage or feeding of insects or rodents. PIC will have facility treated for pests

Observed microwave in room 138 with a heavy accumulation of rust. CA: All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin. PIC will replace microwave

Observed holes in sheets in room 139. CA: Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. PIC will replace bedding

Observed dead pests in storage rooms. PIC will have the dead pests removed

Observed holes in the curtains in room 124. PIC will have the curtains replaced
