Upon arrival, observed a frozen bag of fries stored inside the handwashing sink. CA: A handwashing sink shall be maintained so that it is accessible at all times for employee use. COS: PIC removed the bag of fries from the sink.

Observed the indirect plumbing floor-drain slow draining. CA: A plumbing system shall be repaired according to law; P and maintained in good repair. PIC stated they are going to have their grease trap company come next week and will have it snaked. Advised to send the health inspector a copy of service report by 5/6/26.

Observed a large container of uncooked white rice adulterated with multiple live insects (possibly weevils). PIC stated they had just bought the rice and that it came from the vendor like that. CA: Food shall be safe, unadulterated, and honestly presented. COS: PIC discarded the whole container of rice and placed the container near the 3-compartment sink to be cleaned and sanitized.

Observed a personal beverage, with no lid or straw, stored on the prep table in the kitchen area. CA: Employees shall consume food only in approved designated areas separate from food preparation and serving areas, equipment or utensil areas and food storage areas. However, drinking from a single service beverage cup with a secure lid and straw that is handled to prevent contamination of the employee’s hands, the container, exposed food, clean equipment, utensils and linens, unwrapped single-service and single-use articles will be allowed. COS: PIC removed the personal beverage from the kitchen area.

Observed a fly swatter stored on a shelf amongst containers of sauces. CA: Insect control devices shall be installed so that: (i) The devices are not located over a food preparation area; and (ii) Dead insects and insect fragments are prevented from contact with exposed food; clean equipment, utensils, and linens; and unwrapped single-service and single-use articles. COS: PIC voluntarily discarded the fly swatter.

Observed a container of raw chicken stored on a shelf directly above a container of cooked peas and carrots. Advised on how to store foods properly, based on the food item's minimum cook temperature. Ready-to-eat foods shall be stored above raw animal foods. COS: PIC moved the raw chicken to a different cooler.
