Observed employee handling and serving food with painted fingernails.

CA: (f) Fingernails. Employees shall keep their fingernails clean and trimmed to no longer than the tips of the fingers. Pf Unless wearing gloves in good repair, a food employee may not wear fingernail polish or artificial fingernails when working with exposed food. Pf

COS:Employee was instructed to discontinue food handling and properly wash hands. Employee then put on gloves to prevent contamination.

Observed no paper towel at the second hand sink by the ice machine. CA: Each handwashing sink or group of adjacent handwashing sinks shall be provided with: Individual, disposable towels; A continuous towel system that supplies the user with a clean towel. COS: Paper towel was provided.

Observed food employee with hair more 1/2 inch without proper restraint. CA: Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch. COS: Hair was pulled back and restraint properly.

Observed Floors underneath equipment and along the walls to be unclean with debris and food accumulation. CA: The physical facilities shall be cleaned as often as necessary to keep floors and walls clean and by methods that prevent contamination of food products.

Observed "pre cooked" bacon on top of raw shelled eggs and next to ready to eat eggs and bread. CA: Food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from, Raw ready-to-eat food including other raw animal food such as fish for sushi or molluscan shellfish, or other raw ready-to-eat food such as fruits and vegetables, Cooked ready-to-eat food. COS: Bacon was removed from the raw shell eggs.

Observed employee wearing jewelry while prepping food.CA: Jewelry. Except for a plain ring such as a wedding band food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food. COS: Jewelry was removed and employee washed hands.

Observed soda nozzle at the drive thru and front service area unclean to sight and touch. COS- nozzle was removed washed and sanitized. Advised to increase cleaning frequency.

Observed 2 employees wearing bracelets while preparing food. COS- employees removed arm jewelry washed and sanitized hands.

Observed both restrooms unclean to sight. COS- Employee cleaned restrooms.
