Observed pet water bowl in main waiting area upon entering. PIC stated pets were allowed inside dining area of facility. Facility has no written pet policy. CA. PIC advised that Live animals may not be allowed on the premises of a food service establishment. Live animals may be allowed in the following situations if the contamination of food, clean equipment, utensils, and linens; and unwrapped single-service and single-use articles cannot result:

Observed the PIC arrived in the facility without washing hands and began prepping food. PIC was asked to washed hands, however, he did not follow handwashing guidelines. There were 2 other observations of the PIC not washing hands before donning gloves and then handling food.
