Facility lacking certified food safety manager. CA: Food service establishments shall have in its employ a Certified Food Safety Manager (CFSM). Person-in-charge shall provide Servsafe certificate course information to Dekalb Public Health via email within 30 days.
10D - food properly labeled; original container

Observed sugar removed from its original packaging placed into three working containers without its common name at the coffee station. Observed a single-use container with unidentifiable seasoning not bearing its common name. CA: A working or bulk container shall have the common name of the food, or if there is no common name, an adequately descriptive identity statement.
15B - warewashing facilities: installed, maintained, used; test strips

Facility lacking test strips to measure concentration of chlorine. A test kit or other device that accurately measures the concentration in mg/L of sanitizing solutions shall be provided. Provided person-in-charge chlorine test strips.
