Observed food stored uncovered in prep top cooler and freezer. CA: (iv) Except as specified under paragraph 2. of this subsection and when cooling as specified in DPH Rule 511-6-1-.04(6)(e)2.(ii), storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination;
4-2B - food-contact surfaces: cleaned & sanitized

Observed cell phone stored on food contact surface ( cutting board). CA:1. Equipment food-contact surfaces and utensils shall be cleaned At any time during the operation when contamination may have occurred. COS- cell phone removed. PIC instructed to wash, rinse and sanitize surface

Observed vegetable sink unclean with accumulation of black build up. CA:(a) Equipment, Food-Contact Surfaces, and Utensils.
- Equipment food-contact surfaces and utensils shall be clean to sight and touch.
6-2 - proper date marking and disposition
Regulation: 511-6-1.04(6)(h) - ready-to-eat time/temperature control for safety food, disposition (p)
Observed 2 containers of cooked chicken with discard date of 4/30/26 in walk in cooler. Observed large container coleslaw with discard date 5/10/26. CA: Ready-to-Eat, Time/Temperature Control for Safety Food, Disposition.
- A food that requires datemarking shall be discarded if it:
(i) Exceeds 7 days, not including the time that the product is frozen; P
(ii) Is in a container or package that does not bear a date or day; P or
(iii) Is inappropriately marked with a date or day that exceeds 7 days . P
- Refrigerated, ready-to-eat, time / temperature control for safety food prepared in a food service establishment and dispensed through a vending machine with an automatic shutoff control shall be discarded if it exceeds 7 days. COS- PIC voluntarily discarded food items.
18 - insects, rodents, and animals not present

Observed gap on the bottom and top of back door. CA:511-6-1.07(2)(m) - Outer Openings Protected (C) (m) Outer Openings, Protected.
- Except as specified in paragraphs 2 through 5 of this subsection, outer openings of a food service establishment shall be protected against the entry of insects and rodents by:
(i) Filling or closing holes and other gaps along floors, walls, and ceilings;
(ii) Closed, tight-fitting windows; and
(iii) Solid, self-closing, tight-fitting doors.
