Observed mold growth on ceiling of bathroom in Room 112 likely due to ventilation fan not properly working. Owner stated that entire hotel is being remodeled room by room and that ceiling would be cleaned and painted and that fan would be replaced or repaired as needed

Observed unrepaired damage to ceilings, walls, and floors in several rooms. Observed damage to walls in room 220 and a screw sticking through wall from a previous attempt to repair the shower curtain rod in room 218. Observed damage to ceiling in room 222. PIC stated that screw and damage to ceilings and walls would be repaired as soon as possible

PIC could not produce current gas fired equipment inspection repair. PIC has stated owner will locate inspection report and email to EH today. If gas fired equipment inspection report is emailed to EH today, violation will be removed. If gas fired equipment inspection cannot be located or performed in 2024, have gas fired equipment inspected within 30 days. Facility should keep inspection report easily accessible for PIC to locate

Observed debris/dust buildup behind beds along floor/wall juncture in several rooms. PIC is to discuss with housekeeping staff. All furniture in rooms to be cleaned under and behind as guests checkout and rooms become available.
Observed wall in Room 118 having spattering and staining behind bed and refrigerator/desk area. PIC is to discuss with housekeeping staff to clean wall to sight and touch before next guest checks in.
Observed bathroom wall in Room 215 having been patched but not painted. PIC stated she will discuss with maintenance staff about painting wall. Wall to be painted within 7 days.
Observed bathroom ceiling in Room 419 peeling and flaking from ceiling. PIC is to have maintenance staff inspect ceiling and determine effective action. Repair ceiling within 30 days

PIC, Michelle, could not produce where gas fired equipment has been inspected within last year. If gas fired equipment inspection can be faxed or emailed to EH by 05/29/24, 12:00 PM, inspection report will be amended. If gas fired equipment inspection has not been performed in 2024, inspection to be performed within 30 days. Phone number and email address provided to PIC

Observed microwave in Room 105 had not been inspected nor cleaned since last occupied. At time of inspection, foam plate and two packages of sausage biscuits were found in microwave. Plate and biscuits were removed and discarded by PIC. Manager/PIC is to instruct housekeeping staff to check and clean all furnishings and appliances after guest checkout

Observed gold bed coverings for both beds in Room 220 appeared to have excessive marks and staining and would not meet guest expectations of clean. If bed coverings cannot be washed and cleaned to where stains are not present, discard bedspread and replace by next guest

Observed that wall between guest bath and bed appeared dingy/dirty and stained. Would not meet guest expectations of clean. Wall is to be scrubbed and cleaned to sight and touch. If wall still appears dirty and stained after cleaning, repaint wall within 30 days. See Pictures of Room 220.

Other room entry doors were not dingy and stained like Room 220

Observed dirt/debris on bed mattress under mattress pad in Room 313. PIC was shown dirt/debris. Room was taken out of rotation until housekeeping can vacuum and clean bed to sight and touch

Observed excessive mold and mildew at shower pan and wall surround in Room 210. Discussed mildew/mold cleanup with PIC. Mold/Mildew is to be cleaned to sight or stripped and recaulked within 7 days

Repeat Violation: Facility could not produce gas fired inspection report that shows gas fired equipment has been inspected within last year. EH inspection report dated 11/29/22 noted facility was to have gas fired equipment inspection performed within 10 days. Inspection was not performed. Facility is to have gas fired equipment inspection performed and emailed to EH by close of business 09/26/23

Observed stains and holes (left bed) on yellow top bed covering in Room 403. Observed ripped mattress pad cover in Room 203. Any bed linens having excessive stains, rips, or holes are to be taken out of service. Observed desk/office chair provided in Room 203 to be almost completely stripped of vinyl covering and exposing fabric backing. Only small area of vinyl was left on chair. Chair cannot be repaired and is to be discarded and replaced with a chair in good repair. Replace chair in Room 203 within 7 days

Repeat Violation: Most recent tourist accommodation inspection report (11/29/22; 87) nor tourist accommodation operational permits were not posted as prescribed in rules. Post operational permit in public view. Post 09/12/23 inspection report in public area such as area near registration desk (same as swimming pool inspection report) by 5 PM today
Regulation: 511-6-2-.08(4) - toilet facilities-cleaning and maintenance
Observed bathtubs in Rooms 403 and 409 having slow draining of water in tub. Water took over five minutes to drain completely in Room 409. Facility is to have maintenance personnel inspect tubs and slow draining by 5PM today. If slow draining cannot be corrected today, rooms should not be rented until repaired
