Observed employee switching tasks, putting on gloves without washing hands to include drinking. CA: Ensure proper handwashing procedures.

Regulation: 511-6-1.04(1) - safe, unadulterated and honestly presented (p)
Observed cabbage/lettuce in WIC rotten/spoiled. COS: Item was discarded. CA: Check food items for spoilage.
6-1A - proper cold holding temperatures
Regulation: 511-6-1.04(6)(f) - time/temperature control for safety; cold holding (p)
Observed WIC cold holding at 72 degrees F and items in cooler above 41 degrees F. COS: Items moved to ice and refrigerated truck ordered. CA: Don't use WIC until colding holding is 41 degrees F and below.
6-1C - proper cooling time and temperature

Observed cooling of chicken starting at 113 degrees F and after 1.5 hours only at 107 degrees F. COS: Items were discarded. CA: Ensure proper cooling methods.

Observed the establishment with multiple foodborne illness risks to include the PIC not following managerial duties to include holding temps., handwashing, cooling, thawing. CA: A PIC with active managerial control and responsibility should be put in place.

Observed 3 employees without proof of Employee Health Reporting Agreement. CA: Ensure all employees have agreement.

Observed employees drinking from drinks without proper lid and straw and in kitchen. CA: Ensure cups with lid and straw are used.

Observed handsink without working soap dispenser. CA: Fix soap dispenser and obtain soap.

Observed several food items stored uncovered throughout kitchen (chicken, fish, onions, shrimp, etc.). CA: Cover all food items.

Observed establishment storing food in metal cans. CA: Food should not be kept in metal cans after opening and once moved to storage container it should be labeled. COS: Cans were discarded.
