Food-contact surfaces not cleaned between uses with fruits and vegetables and potentially hazardous foods. Observed employee use tomato slicer and not was, rinse, or sanitize after use before replacing it to its storage area.

Observed employees personal drinks stored near water source below Dr k lids an in reach in coolers intended for food storage use. PIC removed.
12B - personal cleanliness

Observed employee at veggie prep area slicing tomato with no hair restraint
12C - wiping cloths: properly used and stored
Regulation: 511-6-1.04(4)(m) - wiping cloths, use limitation (c)
Observed wet wiping cloths stored on back food prep counter near the meat slicer and not in sanitizer solution between use.
16B - plumbing installed; proper backflow devices
Regulation: 511-6-1.06(2)(r) - system maintained in good repair (p, c)
Observed cold water at employee hand sink shut off because when left on it leaks. Discussed with PIC this should be repaired so that the sink is useable.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed ceiling tiles in dining area in poor repair with water stains
