Observed orange slime build up in ice bin in kitchen area beside RIF. CA: Equipment food-contact surfaces and utensils shall be cleaned such as ice bins. PIC will drain, clean, and sanitize ice bin in kitchen area beside RIF.

Observed color coded prep boards with visible cuts and stains. CA: Equipment and utensils shall be designed and constructed to be durable and to retain their characteristic qualities under normal use conditions. PIC will buy more color coded prep boards.

Observed microwave in kitchen area accumulated with food debris. CA: Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. PIC will clean microwave in kitchen area with food debris.

Observed no chemical sanitizer solution being detected on the test strips in the dishwasher. CA: Manual and Mechanical Warewashing Equipment, Chemical Sanitization - Temperature, pH, Concentration, and Hardness. A chemical sanitizer used in a sanitizing solution for a manual or mechanical operation at contact times specified under subsection (8)(b)3 of this Rule shall meet the requirements specified in DPH Rule 511-6-1-.07(6)(g), shall be used in accordance with the Environmental Protection Agency (EPA)-registered label use instructions P, and shall be used as follows: A quaternary ammonium compound solution shall have a minimum temperature of 75ºF (24ºC), P have a concentration as specified in DPH Rule 511-6-1-.07(6)(g) and as indicated by the manufacturer’s use directions included in the labeling, P and be used only in water with 500 ppm hardness or less or in water having a hardness no greater than specified by the EPA-registered label use instructions. PIC will have sanitizer repaired to working order.

Observed rice, pork, and beans out of the original containers not properly dated marked in walk in cooler. CA: Except when packaging food using a reduced oxygen packaging method, and except as specified in paragraphs 4 and 5 of this subsection, refrigerated, ready-to-eat, time/temperature control for safety food prepared and held in a food establishment for more than 24 hours shall be clearly marked to indicate the date or day by which the food shall be consumed on the premises, sold, or discarded, when held at a temperature of 41°F (5°C) or below for a maximum of 7 days. The day of preparation shall be counted as Day 1. PIC is in the process of properly date marking rice, beans, and pork that are out of the original containers in the walk in cooler.

Observed hot water not working properly at employee handwashing sink in the employee restroom. CA: Hot water generation and distribution systems shall be sufficient to meet the peak hot water demands throughout the food service establishment. PIC will have hot water repaired at employee handwashing sink in employee restroom.

Observed water pressure lacking at sink in women restroom. CA: Water under pressure shall be provided to all fixtures, equipment, and nonfood equipment that are required to use water except that water supplied as specified under subsection (1)(k)1 and 2 to a temporary food service operation or in response to a temporary interruption of a water supply need not be under pressure. PIC will have water pressure repaired at sink in women restroom.

Observed no employee handwashing sign at employee handwashing sink in kitchen area. CA: A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees and shall be clearly visible to food employees. PIC will be putting up an employee handwashing sign at employee handwashing sink in kitchen area.

Observed raw chicken and raw steak in the prep top cooler out of temp. PIC stated that the meat has been in the prep top cooler since yesterday. CA: ime/Temperature Control for Safety Food, Hot and Cold Holding. Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F (5°C) or below. COS PIC tossed product in the trash.

Observed several cutting boards in the facility that have deep cut marks and do not look clean to sight. CA: Equipment food-contact surfaces and utensils shall be clean to sight and touch. PIC will have cutting boards replaced with ones that are in good repair.

Observed the prep top cooler prep board with deep cuts and not clean to sight. CA: Equipment food-contact surfaces and utensils shall be clean to sight and touch. PIC will have prep top board replaced with one in good repair.

Observed a leak at the vegetable sink faucet. CA: A plumbing system shall be repaired according to law; P and maintained in good repair. PIC will have leak on the faucet repaired.

Observed a bowl being used as a scoop in the beans container in the WIC. CA: uring pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored: in the food with their handles above the top of the food and the container. COS PIC removed the bowl from the beans container.

Observed dishwasher not properly dispensing sanitizer solution. CA: A warewashing machine that is installed after September 12, 2007 shall be designed and equipped to: automatically dispense detergents and sanitizers Pf and incorporate a visual means to verify that detergents and sanitizers are delivered or a visual or audible alarm to signal if the detergents and sanitizers are not delivered to the respective washing and sanitizing cycles. PIC will have dishwasher repaired to working order.

Observed light blown in the dry storage/dishwasher area. CA: At least 20 foot candles (215 lux):At a distance of 30 inches (75 cm) above the floor in areas used for handwashing, warewashing, and equipment and utensil storage, and in toilet rooms. PIC will have light repaired to working order.

Observed black mold build up on drink nozzle machine in dining area. PIC will have drink nozzle machine cleaned and sanitized by today. PIC stated that she will make sure the drink nozzles are cleaned and sanitized each day before closing.

Observed wet wiping cloths being stored in vegetable sink in main kitchen beside employee handwashing sink. PIC removed wet wiping cloths from vegetable sink in main kitchen beside employee washing sink and stored them properly in wiping cloths bucket.

Observed loose faucet at employee handwashing sink near employee entrance door before entering main kitchen. PIC will have until 11/17/23 to have lose faucet at employee handwashing sink near employee entrance door before entering main kitchen repaired.

Observed handles down in rice and sugar bins in back area of facility. PIC put handles up in rice and sugar bins in back area of facility.

Observed accumulation of grease under vent hood in main kitchen. PIC will clean vent hood in main kitchen.

Observed blown light bulb in middle of vent hood. PIC will have until 11/17/23 to have blown light bulb in middle of vent hood replaced.
