Observed employees were not being trained on food allergy awareness. C/A Food Safety Training. Employees are properly trained in food safety, including food allergy awareness, as it relates to their assigned duties. Food allergy awareness includes describing foods identified as major food allergens and the symptoms that a major food allergen could cause in a sensitive individual who has an allergic reaction;Pf COS: EHS PROVIDED THE FACILITY WITH A ALLERGEN AWARENESS LOG/FORM AND HAD THE EMPLOYEES READ AND SIGN THE FORM

Observed employees not being aware nor had a verifiable manner of which which are the 5 symptoms and 6 illnesses they have to report to the PIC C/A Reporting Responsibilities. Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with the Chapter, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food;Pf COS COS EHS HANDED THE PROPER FORMS TO MANAGER AND HAD THE EMPLOYEES READ AND SIGN THEM
4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(6)(n) - manual and mechanical warewashing equipment, chemical sanitization-temperature, ph, concentration, hardness (p,pf)
OBSERVED THE DISHWASHER MACHINE AT THE BAR WITHOUT ANY SANITIZING LIQUID. MANAGER STATED THEY PLACED AN ORDER ALREADY. C/A A chlorine solution shall have a minimum temperature based on the concentration of 50-100ppm and QUAT 200-400ppm COS EHS instructed staff to wash bar glasses at the three-compartment sink or at the large dishwasher in the kitchen
4-2B - food-contact surfaces: cleaned & sanitized

Observed significant mold build up on the ice bin chute in the main kitchen C/A Cleaning of Equipment and Utensils.

- Equipment food-contact surfaces and utensils shall be clean to sight and touch. Pf. COS Manager emptied the ice bin and clean the chute before EHS left the premises
14B - utensils, equipment and linens: properly stored, dried, handled
Regulation: 511-6-1.05(10)(e)1,2,4 - equipment, utensil, linens, stored 6" off floor in clean, dry location (c)
Observed ALL ceramic serving dishes were stored uncovered and open on the shelves throughout the main kitchen. C/A ALL DISHES SHOULD BE COVERED OR STORED UPSIDE TO PREVENT CONTAMINATION COS: ALL DISHES WERE TURNED UPSIDE DOWN AND THE SINGLE TO GO CONTAINERS WAS ADVISED TO KEEP THEM IN THE PLACTIC BAGS
