PIC unable to provide a verifiable manner in which food employees have been informed of their responsibility to report to the person in charge (PIC) as it relates to foodborne illness.
C/A: Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with the Chapter, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food
COS: EHS provided a employee health policy to only food employee present
2-2D - adequate handwashing facilities supplied & accessible
Regulation: 511-6-1.06(2)(c) - handwashing sinks, installation (pf, c)
Observed the hottest water temperature for both hand sinks in the bathrooms (men and women's) at 55F. Observed the hottest water temperature for the only hand sink in the main kitchen at 57F.
C/A: A handwashing sink shall be equipped to provide tempered water at a temperature of at least 85ºF (29.4 ºC) through a mixing valve or combination faucet.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed the toilet in the men's bathroom unable to flush. Observed a large hole in one of the booths in the main dining area.
C/A: All physical facilities shall be maintained in good repair.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(n) - maintaining premises, free of unnecessary items/litter (c)
Observed 5 old frozen yogurt machines that are no longer in use located in the main kitchen Observed a 3-door Reach-in cooler no longer in use located in the main kitchen.
C/A: The premises shall be free of items that are unnecessary to the operation or maintenance of the establishment, such as litter or equipment that is nonfunctional or no longer used and litter.
