Observed facility operating without employee health agreement forms. Advised to obtain forms and have employees sign.
2-2D - adequate handwashing facilities supplied & accessible
Regulation: 511-6-1.06(2)(o) - using a handwashing sink- operation & maintenance (pf)
Observed hand sink in the main kitchen with food particles in the basin of the sink. Advised PIC hand sinks shall be used for handwashing only.
2-2E - response procedures for vomiting & diarrheal events
Regulation: 511-6-1.03(6) - clean-up of vomiting and diarrheal events(pf)
Observed facility operating without cleanup procedures when responding to vomiting/diarrhea events. Advised to obtain clean up procedures.
16A - hot and cold water available; adequate pressure
Regulation: 511-6-1.06(1)(g),(h) - water supply, capacity; pressure (pf)
Observed no hot water available at both hand sinks in the main kitchen and the front food service area. Advised hot water shall be available during peak operational times. COS: Hot water was restored.
16B - plumbing installed; proper backflow devices
Regulation: 511-6-1.06(2)(r) - system maintained in good repair (p, c)
Observed leak at main kitchen hand sink. Advised PIC plumbing shall be maintained in good repair at all times.
