Observed cloudy water in active use, not able to see main drain covers. Advised PIC Water clarity shall be maintained. When standing at the pool's edge at the deep end, the main drain suction outlet covers or a four inch by four inch square marker tile in contrasting color shall be clearly visible. When standing at a spa's edge, the deepest portion of the spa floor shall be visible when the water is still. 1. For pools over ten feet deep an eight inch by eight inch square marker tile in a contrasting color to the pool floor shall be visible at all deepest part of the pool. 2. This reference point shall be visible at tall times from the edge of the deck times from the edge of the deck. CA: Had all swimmers (3) exit pool immediately, pool voluntarily closed.

Observed emergency phone, difficulty dialing to 911, took multiple attempts. Advised PIC An operable, hard-wired, conventional telephone line or continuous powered source, weatherproof emergency phone shall be permanently installed in a conspicuous location within the pool enclosure and must be readily available to bathers at all times. The emergency phone shall be capable of connecting to 911 and electronically transferring an automatic number identification and automatic locator identification of the pool emergency phone to the public safety answering point, if available.

Observed bromine levels of pool at 2.0 ppm. Advised PIC During operation, disinfectant levels are less than the minimum level specified in DPH Rule 511-3-5-.17. If the level of the disinfectant used is not specified in DPH Rule 511-3-5-.17, the disinfectant must be approved and kept at levels determined necessary by the health authority.

Upon arrival, observed pH levels reading 7.0. Advised PIC that pH levels for pool shall be between 7.2-7.8. COS: CPO increased level of pH to 7.8.

Observed inadequate Warning, No Lifeguard on Duty signage located on the outside portion of the pool. Advised PIC/CPO that Where no lifeguard is on duty, a sign or signs shall be placed in clear view at or near the entrance to the pool and shall state in clearly legible letters at least four inches high “WARNING - NO LIFEGUARD ON DUTY and RISK OF DROWNING - SUPERVISE CHILDREN CLOSELY”. COS: PIC placed adequate signage on the outside portion of the pool.

Observed bromine levels of pool at 1 ppm. Advised PIC bromine levels shall be adequate (3 to 8) ppm. During operation, disinfectant levels are less than the minimum level specified in DPH Rule 511-3-5-.17.

Observed pH reading of 7.0 at time of inspection. Advised PIC during operation, the pH is less than the minimum or more than the maximum levels allowed in DPH Rule 511-3-5-.l7.

Observed loose grab rails at depths 4ft and 5ft (inside and outside). Advised PIC handrails, if removable, shall be installed in such a way that they cannot be removed without the use of tools.

Observed No, "No Diving" tiling at depths of 3ft, 4ft, and 5ft. And No Universal no Diving Tiles at 4ft, 5ft. Observed no Depth marker of 5ft (inside and outside) at change of elevation. Advised PIC The words "No Diving" and the universal international symbol for “NO DIVING” shall be permanently visible at the edge of the deck for water five feet in depth or less, placed only on the deck beside the depth markers, and shall conform to that outlined for depth markers in subsections (3)(a) thru (h) above.

Observed no pool risks sign, pool rules posted in outside enclosure area of pool. Advised PIC The same sign in subsection (b) above or an additional sign will state under the heading “Pool Risks” the following items in clearly legible letters at least one inch high.

Observed no deck shower at time of inspection. Advised PIC a minimum of one rinse shower shall be provided on the pool deck of all public pools and spas. Water used for rinse showers may be at ambient temperature.

Observed most recent pool inspection report not posted in public display. Advised PIC The operator shall receive a copy of the inspection and place it in a location protected from the weather and in public view as designated by the health authority.

Observed universal no diving tiling not installed at each depth marker throughout the facility. Advised PIC that universal no diving tiling/marker shall be permanently installed at each depth marker on the pool deck.

Observed inadequate no lifeguard on duty signage at the outdoor portion of the pool deck during time of inspection. Advised PIC Where no lifeguard is on duty, a sign or signs shall be placed in clear view at or near the entrance to the pool and shall state in clearly legible letters at least four inches high “WARNING - NO LIFE GUARD ON DUTY and RISK OF DROWNING - SUPERVISE CHILDREN CLOSELY".

Observed insufficient throw rope length at time of inspection for outside decking area of pool not meeting one and one-half the width of the pool. Advised PIC A minimum one-fourth inch diameter throwing rope one and one-half times the maximum width of the pool or fifty feet in length, whichever is less, to which has been firmly attached a ring buoy with an outside diameter of approximately fifteen inches or a similar flotation device which is U.S. Coast Guard approved

Observed painted depth markers plaster on pool deck. Advised PIC Depth Markers for Spas. Public spas shall have permanent depth markers with numbers and letters a minimum of four inches high plainly and conspicuously visible from all obvious points of entry.

Observed no universal no diving sign throughout pool deck (indoor and outdoor). Advised PIC The words "No Diving" and the universal international symbol for “NO DIVING” shall be permanently visible at the edge of the deck for water five feet in depth or less, placed only on the deck beside the depth markers.

Observed no hours posted and no theoretical peak occupancy posted at outside area of pool. Advised PIC A sign shall be posted stating the hours of operation in clear view near the entrance and shall state the theoretical peak occupancy.

Observed signage on outside area of pool bearing "WARNING: NO LIFEGUARD ON DUTY." Advised PIC Where no lifeguard is on duty, a sign or signs shall be placed in clear view at or near the entrance to the pool and shall state in clearly legible letters at least four inches high “WARNING - NO LIFE GUARD ON DUTY and RISK OF DROWNING - SUPERVISE CHILDREN CLOSELY”

Observed no direction of flow marking on pipes in pool pump room. Advised PIC All piping shall be marked with directional arrows as necessary to determine flow direction. All piping in the equipment room shall be permanently identified by its use and the pool and or aquatic feature it serves.

Observed painted depth markers plaster on pool deck. Advised CPO/PIC that depth markers shall be permanently displayed on pool deck.

Observed no universal no diving sign throughout pool deck. Advised PIC/CPO that universal no diving markers shall be placed on pool deck.

Upon arrival, observed most recent swimming pool inspection report not posted in public view. Advised CPO/PIC that the most recent inspect report shall be posted at all times.

Missing 1 weir. Advised CPO must replace.

Observed no daily self-checks being kept. Advised PIC must perform and keep. Provided form via e-mail.

Water cloudy; could not see main drains at various points of pool.

Observed the turnover rate for the pool insufficient in reaching the required 65 GPM for a 6 hour turnover. CA: repair to ensure proper turnover for the pool (per the design flow rate).

Observed both weirs missing.
