Observed ice machine lid at drive thru window with brown/black substance. Advised PIC that all food contact surfaces shall be clean to sight and touch.
Observed multiple sanitizer buckets throughout main kitchen measuring at 150 ppm Quat. Advised PIC that when using quat as a sanitizer the concentration range shall read betweenv200-400 ppm. Advised PIC to remake sanitizer solution. COS: PIC had food employee remake 3 sanitizer buckets to read 300 ppm Quat.

Observed multiple food employees prepping food without a hair restraint. Advised PIC that hair restraints shall be worn when handling food and in dish washing areas. COS: PIC had food employees wear hair restraints.
