PIC could not demonstrate in a verifiable manner that food employees are/were informed of their responsibility to inform regarding foodborne illnesses/symptoms. Advised PIC that an employee health policy and agreement forms shall be produced upon request of the Health Authority. Advised PIC to obtain an employee health policy and agreement forms and to maintain on file.
2-2D - adequate handwashing facilities supplied & accessible
Regulation: 511-6-1.07(3)(b) - hand drying provision (pf)
Upon arrival, observed no paper towels in main kitchen hand sink and men's restroom hand wash sink. Advised to place paper towels. CA: Hand wash sinks shall be supplied with paper towels.

Observed hand wash sink in main kitchen blocked by dishes. Advised must keep unblocked and that hand wash sink is for hand washing only. CA: Hand wash sink shall be kept unblocked and hand wash sinks are for hand washing only.

Observed food contact surfaces of the white panel of the ice machine in the main kitchen stored as clean with buildup and in need of cleaning. Advised to clean. CA: All food contact surfaces shall be kept clean.

Upon arrival, observed the front door open. Advised to close. CA: All outer openings shall be kept secure.

Observed most recent inspection report not posted. Advised to post most recent inspection in public view. CA: Most recent inspection report shall be posted in public view.

Observed scoop with handle down in coffee. Advised to move with handle facing up. CA: Properly store in use utensils between use.

Observed heavy build up inside reach in freezer in main kitchen. Advised to clean. CA: Non-food contact surfaces shall be kept clean.
