PIC could not produce signed employee health reporting agreements. Advised they must be kept on file at facility so they can be reviewed in a verifiable manner. CA: Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with the Chapter, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food

Observed no paper towels at main kitchen hand wash sink and men's restroom. COS: PIC placed paper towels. CA: Paper towels shall be provided at all hand wash sinks.

Observed no soap at main kitchen hand wash sink. COS: PIC placed soap. CA: All hand wash sinks shall be supplied with soap.

Observed grocery bags be used to store foods. Advised must be food grade bag or container. CA: Use food grade containers to properly store food.

Observed 2025 operating permit not displayed. Advised to post. CA: Current operating permit shall be posted in public view.

Observed employee personal belongings throughout main kitchen. Advised must keep in designated area. CA: Keep employee personal items in designated ares.
