Regulation: 511-6-1.05(6)(n) - manual and mechanical warewashing equipment, chemical sanitization-temperature, ph, concentration, hardness (p,pf)
Quaternary ammonium sanitizer in in-use buckets and 3-compartment sink at 0 ppm. Corrective action (CA): Quaternary ammonium sanitizer concentration must be maintained at range specified by manufacturer. Person in charge (PIC) hand mixed buckets & 3-compartment sink to the correct concentration until maintenance can fix dispenser.

Observed mold-like substance in bulk ice machine. CA: Clean ice machine frequently enough to preclude growth.

Ice cream and raw chicken stored on floor of walk-in freezer. PIC moved food items up on shelving.
