4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(7)(b) - food contact surfaces and utensils - cleaning frequency (p, c)
Observed substantial pink and black mold build-up along interior portions of bulk ice machine, specifically in ice chute and upper area where ice is formed. Ice machine must be emptied, cleaned, and sanitized within 72 hours. Manager stated that ice machine is cleaned quarterly by an outside company but must increase cleaning frequency to a rate that precludes mold accumulations.
Use of ice was discontinued and bagged ice was purchased.
4-2B - food-contact surfaces: cleaned & sanitized

Observed:
- Several pans stored as clean but visibly dirty on dish shelve to left of deli slicer
- Significant food build-up on deli slicer. Slicer was not used today and was not cleaned after previous use.
Equipment food-contact surfaces and utensils shall be clean to sight and touch. Must rewash dirty pans and clean slicer.
18 - insects, rodents, and animals not present
Regulation: 511-6-1.07(5)(k) - controlling pests (pf, c)
Observed live roach crawling around fryer area and dead roaches behind fryer. Staff is aware of issue and has had pest control come out to treat. The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by:

Routinely inspecting incoming shipments of food and supplies;

Routinely inspecting the premises for evidence of pests;

Using methods, if pests are found, such as trapping devices or other means of pest control as specified under subsections (6)(e), (6)(m), and (6)(n) of this Rule; Pf and
-
Eliminating harborage conditions.
Must increase pest control frequency and pull out all equipment in area for deep cleaning to deter harborage.
15A - food and nonfood-contact surfaces cleanable, properly designed, constructed, and used
Regulation: 511-6-1.05(6)(a) - good repair & proper adjustment (c)
Observed broken handle on prep top lid of salad unit and a zip tie being used as the handle. Equipment shall be maintained in a state of repair and condition that meets the requirements specified under subsections. Handle must be replaced.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(d) - nonfood-contact surfaces (c)
Observed substantial mold build-up on exterior of beer lines in beer cooler. Must also clean shelves and drain bucket. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. Must clean.
