Observed a large container of chicken salad located in the prep top cooler marked with a prep date of (9/11/2025) that is past the 7 day use by date. CA: A food that requires date marking shall be discarded if it exceeds 7 days. Example foods prepared on 9.11.2025 shall be expired on 9.17.2025. COS by discarding food item.

Observed missing signed employee health policy forms for a large number of staff where employees acknowledge they are aware of the illnesses/symptoms they must report to the Person-In-Charge (PIC). Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with the Chapter, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food.

Observed two containers containing white vinegar and quats not labeled. Working containers used for storing poisonous or toxic materials such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material. COS by adding labels to the containers

Observed employee preparing food while wearing a watch. Employees shall be free of jewelry except for a plain ring such as a wedding band food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food. This was noted on the last inspection. COS by removing watch.
