Regulation: 511-6-1.05(6)(n) - manual and mechanical warewashing equipment, chemical sanitization-temperature, ph, concentration, hardness (p,pf)
Observed dish machine not dispensing chlorine due to chemical being very low. Chlorine must dispense at 50 ppm. COS by replacing chemicals and priming dish machine. Chlorine then dispensed at 50 ppm.

Observed pink and black mold build-up along interior portions of bulk ice machine. Ice machine must be emptied, cleaned, and sanitized within 72 hours. Must increase cleaning frequency to a rate that precludes mold accumulations.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Epoxy flooring throughout kitchen area is in disrepair. Floors must be replaced with a more durable material; epoxy flooring is approved but not durable. List of other approved flooring was provided during a previous inspection.
