8 - annual gas fired equipment inspection on file; gas units equipped with automatic safety and/or 100% cut-off pilot
Regulation: 511-6-2-.15(5) - heating and fire safety-inspection
Observed facility to not have an annual gas fired equipment inspection on file. RCA: All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency.

15 - ice from approved source; properly handled, supplied and stored; equipment approved/clean
Regulation: 511-6-2-.07(8) - ice supply
Observed a build of of black/pink debris on the baffle of the guest ice machines on the 1st and 3rd floor. RCA: If self-service ice is provided, then it shall be from an approved water supply. The ice machines must be maintained in good repair and capable of being properly cleaned and sanitized according to the manufacturer’s recommendations

Observed multiple spray bottles of chemicals to not be labeled with the common name of it's contents.
CA: Employee labeled the spray bottles.
Rule: Any spray bottle used in cleaning a tourist room or stored on the housekeeping cart shall be labeled with its contents
