OBSERVED ACCUMULATION OF SOIL REISDUE ON ICE MACHINE. CA: EQUIPMENT FOOD CONTACT SURFACES AND UNTENISLS SHALL BE CLEANED WITH A CLEANING SHECULE APPROVED IN CONSIDERATION WITH THE AMOUNT OF FOOD RESIDUE ACCUMULATION AND AT FREQUECNY SPECIFIED BY THE MANUFACTURER. CB: PIC WILL UPDATE SCHEDULE OF CLEANING ICE MACHINE AND INFORM STAFF OF PROCEDURE.

OBSERVED CHEMICAL SOLUTION IN SANITIZER BUCKET NOT MEETING REQUIREMETNS OF DPH FOOD RULE (NOT AN APPROVED SANITIZER AND NO TESTING METHOD). CA: SANITIZING SOLUTIONS USED FOR CLEANING FOOD CONTACT SURFACES SHALL MEET THE REQUIREMENTS OF DPH FOOD RULE AND BE USED IN ACCORDANCE WITH EPA REGISTERED LABELED INSTRUCTIONS. COS: PIC MADE NEW SANITIZER SOLUTION FROM BLEACH MEETING THE REQUIREMENTS OF DPH FOOD RULE.

OBSERVED WALK IN FREEZER WITH EXCESS ICE ACCUMULATION AND TOP OF MICROWAVE PARTIALLY MELTED. CA: EQUIPMENT SHALL BE MAINTAINED IN A STATE OF REPAIR IN ACCORDANCE WITH MANUFACTURER'S SPECIFICATIONS. CB: PIC WILL BEGIN PROCESS OF REPAIR FOR EQUIPMENT.
