Observed that the facility did not have the annual gas fire inspection report available when requested. Facility has 30 days to get our office the report

Observed the ceiling was peeling in the bathroom of room 210, the plaster on the wall in the bathroom of room 217 was cracking, & the plaster on the wall by the bathtub in room 117 was cracking
