4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(7)(b) - food contact surfaces and utensils - cleaning frequency (p, c)
Observed a thick rust debris inside of the ice machine. The part will need to be cleaned and if the rust doesn't comes off then it will need to be replaced. Also observed the dishwasher with thick brown plaque debris on the interior surface. The dishwasher will need to be clean daily. Lastly, observed a black mold-like debris on the interior surface of the lid for the ice bin.
12A - contamination prevented during food preparation, storage, display
Regulation: 511-6-1.04(4)(r) - food storage, prohibited areas (c)
Observed 2 boxes of drink replacement syrup stored with chemicals. Also, observed 1 box stored on the floor. All food items should be stored 6 inches or higher off the floor.
12B - personal cleanliness
Regulation: 511-6-1.03(5)(j) - hair restraints (c)
Observed several employees prepping and handling food items without a hair restraint on.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Observed the top of the dishwasher and a push cart (used for old grease) in need of cleaning due to debris.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(2)(j) - heating, ventilation, air conditioning, system vents, designed to prevent contamination (c)
Observed several HVAC vents throughout the kitchen in need of cleaning due to a thick dust debris and some with rust on them. If rust won't come off then the vents will need to be painted.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(4)(b) - designated areas for employee activity, located to prevent contamination of food, equipment, utensils, linens, & single service articles (c)
Observed employee cup stored on the prep table near an opened food product. All employees item should be in a designated area.
