Observed potentially hazardous foods cold holding with internal temperature above 41F. CA: Potentially hazardous foods must be held at 41F or below. Keep prep top lid closed and keep foods below loading line in the container. COS PIC discarded out of temp foods.

Observed shell eggs held past 4-hour time. Time eggs are removed from cooler and discard time must be marked, and eggs must be used or discarded on or before the discard time. COS PIC discarded eggs and put out new eggs.

Toilet room doors must be kept completely closed. Automatic door closer on men's room is not closing door all the way
