EHS observed the A/C units in rooms 122 and 123 to be separating from the wall, revealing gaps to the the outside that are big enough for pest or insects to enter the rooms.

511-6-2-.14(5) - Condition of Furnishings All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin.

CA: EHS advised the PIC that the A/C units should be repaired/ refixed to the wall so that openings are closed and light gaps not apparent

EHS observed the microwaves in all visited rooms to be in light to medium states of rusting.

EHS observed the interior of the plastic ice bin in room 127 to be dirty and the phone in room 122 not to be working.

EHS observed the following:

• Room 123: no mattress pad; stains on mattress cover and top linen layer; hairs on mattress cover.
• Room 127: hairs on sheets; dark stains on sheets.
• Room 139: stains/ markings on pillow case; brown stains on mattress pad

511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly.

CA: EHS advised the PIC to ensure housekeeping is cleaning rooms thoroughly; if stains can't be removed from linen items then they must be discarded and replaced

EHS observed the following:

• Room 123: peeling paint on toilet paper holder, around drain; toilet bowl staining
• Room 127: brown discoloration around bathtub drain.
• Room 139: peeling paint in the bottom of bathtub.

511-6-2-.08(4) - Toilet Facilities-Cleaning and Maintenance Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Walls, floors, and ceilings shall be constructed of easily cleanable nonabsorbent materials and shall be kept clean and in good repair. While being used by tourists, every surface of a bathtub, shower, shower enclosure, toilet and lavatory, which may come in contact with a person’s body, must be sanitized each day, unless the guest has declined regular tourist room services. If a tourist declines regular tourist room services, the tourist accommodation facility must ensure that these surfaces are cleaned and sanitized at least once per week and between tourists.

CA: EHS advised the PIC that items should be repaired and/or replaced

Observed the need to clean the AC filters/unit and cabinetry holding the REF and microwave in all of the rooms. accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin. EHS spoke with the owner about the need to clean every portion of the room thoroughly

Observed hair/stains/debri on various pieces of bedding in all of the rooms. Washable mattress pads or covers shall be used on all mattresses. Observed damaged or missing shower fixtures in rooms 112, 113, 109, and 103; leak at tub faucet in room 106 and 113; urine on toilet in room 112. nonabsorbent and shall be kept clean and in good repair. EHS informed the owner of the need to address the issues noted

Observed bath vents not working and the need to clean from dust accumulation. Toilet, lavatory and bathing facilities shall be mechanically ventilated. Where ventilation ducts are used, ducts from toilet rooms shall not be connected into return ventilation ducts to any other room. In existing tourist accommodations permitted prior to the adoption of this rule, the current ventilation must prevent odor, mold, mildew, and moisture. All new or major renovation of a tourist accommodation will require mechanical ventilation to be installed to the outside of the building separate from heating and air-condition systems. EHS informed the owner of the need to fix the vents and to clean them

Observed debri on flooring in all of the rooms; damaged wall around wall tissue holder in majority of the bathrooms;cracked shower tiles in room 103;damaged floor entry strip in majority of rooms; damaged wall near bathroom entry in room 112 and 113. Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean. In all new constructed and extensively remodeled residential kitchens used in bed and breakfast inns, coved base molding may be required. Walls and ceilings in residential kitchens shall be easily cleanable and light colored. EHS informed the owner of the need for staff to clean the rooms thoroughly and to make the neccessary repairs to the rooms requiring repairs

EHS observed lack of gas-fired inspection report

EHS observed missing or damaged AC unit filters in rooms 125, 127, 128, 123. EHs also observed threadbare and stained upholstery in all rooms observed.

EHS advised PIC to replace or repair damaged AC unit filters and uholstery. PIC stated that AC units were being ordered in an ongoing remodel and would be replaced whole-cloth along with all upholstered chairs in the facility's guest wing

EHS observed damaged coverlets (Incl. cigarette burn marks and holes) in rooms 129, 128, 126, 123. EHS observed a footprint on a pillowcase in room 129.

EHS advised PIC to replace badly damaged comforters and to have housekeeping address pillowcase issue. PIC stated that linens would be very shortly replaced, as they were on order

EHS observed hot water exceeding 120 degrees Fahrenheit in all rooms observed. Violation:Cold running water under pressure shall be provided to all equipment that uses water. Hot and cold running water under pressure shall be provided to all lavatories, bathing facilities, laundry facilities, and water-using equipment where eating and drinking utensils are washed. In all new tourist accommodations, and where possible in existing tourist accommodations, hot water in all guest rooms shall not exceed 120. CA: adjust water heater to 120 degrees

EHS observed air filters with dust accumulation in all rooms observed and broken air vent in room 115. Violation: all furniture, waste receptacles, draperies, carpets, appliances and accessories shall be clean, in good condition, and free of vermin. CA: clean air filters at frequency to prevent dust accumulation and repair or replace broken air vent

EHS observed ice accumulation in freezer section of refrigerators in rooms 103, 107, and 115. Violation: all furniture, waste receptacles, draperies, carpets, appliances, and accessories shall be kept clean, in good condition, and free of vermin. CA: defrost fridges at frequency to prevent ice accumulation

EHS observed bedding in all rooms observed with stained bedding (pillow cases, sheets, toppers). Violation: all beds, mattresses, and bedding shall be kept clean, in good condition, and free of vermin

EHS observed area around dumpster with furniture and pallets piled up. Violation: Areas surrounding these rooms, enclosures, and containers shall be kept clean and orderly. CA: have waste management provider or someone else clear area and haul off property
