511-6-2-.15(5) - Heating and Fire Safety-Inspection All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency

511-6-2-.14(5) - Condition of Furnishings All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin.

Corrective action: Showed cleaning staff filter and discussed with PIC to have filters cleaned

Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly.

Corrective action: Showed cleaning staff and discussed with PIC items to be cleaned or replaced

Observed most recent inspection not posted to the public. Posted inspection date 03/07/2024 and previous inspection dated 10/20/2025.

511-6-2-.05(7) - Inspection Posting The most recent tourist accommodation inspection report form shall be prominently displayed in public view at all times, between five feet and seven feet from the floor and in a public area such as the registration desk, where it can be read at a distance of one foot away.

Corrective action: Discussed with PIC to remove current inspection and replace with most recent

EHS observed the facility not to have a gas-fired equipment inspection on file, at time of inspection. When requested, PIC stated that the facility owner did not seek-out licensed contractor to perform inspection after the previous Tourist Accommodation inspection.

EHS observed the following items:

• Hair on linen in rooms - 202, 214, and 301
• Stains on linen in rooms - 202, 214, 310, and 309
• Stains on pillow case in room 309

CA: Advised PIC that observed linen and pillow covers are to be cleaned and replaced. If facility are unable to remove stains from linen, items are to be discarded and replaced

EHS observed the most recent TA inspection not to be posted for public display. At time of inspection, the displayed inspection report was dated 3/7/24, however the previous inspection was performed on 06/26/25.

Observed that the facility did not have a current Gas-Fired Inspection Report on file. All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. The most current inspection on file was dated 2022. Have Gas Safety Inspection by July 14,2025

Observed that the outside dumpster side doors had been left open. Dumpster and other outdoor garbage containers are to have their doors kept closed to prevent rodent, birds and other pests from gaining entry. Doors were closed

EHS observed hot water exceeding maximum allowable in rooms 101 (131), 105 (130), 109 (125), 115 (130), 110 (130), 106 (128), 326 (124), 214 (126).

511-6-2-.07(2) - Water Supply and Sanitizing

Hot water in all guest rooms shall not exceed 120F.

EHS advised PIC to adjust boilers down in order to bring temperature into compliance

EHS observed damaged and stained upholstery of chairs in rooms 101, 109, 115, 110.

Corrective action: EHS suggested for housekeeping to wear hairnets to prevent such occurrence, and to inspect linens prior to donning on beds

Violation: Toilet, lavatory and bathing facilities shall be mechanically ventilated. Where ventilation ducts are used, ducts from toilet rooms shall not be connected into return ventilation ducts to any other room. In existing tourist accommodations permitted prior to the adoption of this rule, the current ventilation must prevent odor, mold, mildew, and moisture. All new or major renovation of a tourist accommodation will require mechanical ventilation to be installed to the outside of the building separate from heating and air-condition systems. Inspector note: EHS observed ventilation in bathroom not operating. Corrective action: EHS informed PIC that the ventilation fans should be operable as described in Rule.
