Informed PIC to either have spacers placed or repaired. This must be corrected by 2/3/25

EHS observed ceiling tiles needing to be replace, cleaned, or re-painted. 5) Maintenance and Operation.
(a) Good Repair. All physical facilities shall be maintained in good repair. PIC stated that they will re-paint or replace ceiling tiles. This must be corrected by 2/3/25

Observed facility does not have test strips for sanitizing solution(Bleach).(i) Sanitizing Solutions, Testing Devices. A test kit or other device that accurately measures the concentration in mg/L of sanitizing solutions shall be provided. EHS informed PIC of the need to purchase chlorine test strips.

Observed facility's need to clean both male restrooms as an odor was present at gnats/drain flies were also present; cracked handsink in male restroom closest to the bar. (h) Cleaning of Plumbing Fixtures. Plumbing fixtures such as handwashing sinks, toilets, and urinals shall be cleaned as often as necessary to keep them clean and maintained.PIC stated he would clean the bathrooms as he's been having an issue with maintaining the cleanliness due to patron usage.

Observed facility in need of repairs /cleaning in various areas(i.e restrooms, hole in wall under sink in male restroom closest to the bar, cleaning behind bar(few fruit flies present) tiles throughout the facility, cleaning walls in various areas, damaged coving, cleaning AC vents/ducts). (5) Maintenance and Operation.
(a) Good Repair. All physical facilities shall be maintained in good repair.EHS informed PIC of the need to repair any damaged areas and the need for a deep cleaning.
