Observed multiple roaches during inspection. CA: The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises.

Observed potentially hazardous TCS foods improperly cold holding at above 41F in prep top cooler and RIC (see temp log). CA: Discussed with PIC, items had not been in coolers longer than 4 hours. Ranch and chicken were discarded. Lettuce, cucumbers, cheeses, buffalo dip, canadian bacon, and alfredo sauce were moved to WIC to rapidly cool.

Observed employee pull ready-to-eat sliced red onion from the prep cooler and place on a salad. Onions were directly handled with bare hands. CA: Discussed with PIC. Onions were removed from the salad and new onions were put on with gloved hands.

Observed hand washing sink in the back dish area that did not have hot water available. CA: Discussed with PIC. Hot water was turned off due to a leak. The leak will be repaired and the hot water turned back on.
