Observed inoperable phone in room 112 and 210, smoke detector in disrepair in room 321, batteries removed from the smoke detector, mini fridge cabinet in need or repair or replacement, cracked outlet cover and TV damaged and possibly burned in room 314. CA: All furniture, draperies, appliances, carpets and other accessories in the tourist accommodation must be maintained in good repair and kept clean. Corrections must be made before rooms can be rented out

Observed tears on the comforter in room 112, stain on the bedding in room 220, torn box spring and stain on the bedding in room 321, pen markings on the bedding in room 314. CA: CA: Beds, mattresses, mattress pads and all coverings shall be clean and free of vermin. Corrections must be made prior to renting rooms

Observed toilet with build-up in the basin in need of cleaning, wallpaper peeling near the bathtub, and bathroom vent with dust build up on it in room 112, wallpaper near the bath tub peeling and in disrepair and the light bulb burnt out in the bathroom of room 210, dust build up on the bathroom vent in room 321. CA: CA: Toilet rooms and fixtures shall be kept clean and in good repair. Corrections will be made before rooms are rented out

Observed ice machine on level 1 with black build up on dispenser. CA: Ice machines should be cleaned at a frequency that maintains cleanliness to sight and touch. PIC will assign employee to sanitize the machine

Observed no anti-slip surface or mat in the shower of room 321 (tile only). CA: Anti-slip tubs slip strip, appliques, or slip-proof mats shall be provided and kept clean and in good repair. Add mat or re-finish surface before room is rented

Observed torn curtains in room 321 and batteries missing from the smoke detector in room 314

Observed vent fan in bathroom in room 210 with dust build up and dust build up on the AC vent filters in room 314. CA: Vents will be cleaned and moving forward the frequency of routine cleaning will be increased

Observed exterior door in the stairwell propped open. CA: Exterior openings shall be protected with tight-fitting doors. Discussed monitoring and ensuring doors remain closed

Observed ice machine on level 1 with black build up on dispenser. CA: Ice machines should be cleaned at a frequency in order to maintain clean to sight and touch

Observed rooms 301 and 321 with showers that are tiled and have no non-slip surface in place. CA: Showers will be updated so they have a non-slip surface

Observed several unlabeled working spray bottles of toxic substances. CA: Discussed labelling requirement with PIC; all bottles will be labelled

Observed no mattress protectors in rooms 114, 119, 215, or 313. CA: All mattresses should have a protector to cover all exposed areas of the mattress

Observed AC filters in rooms 119 and 215 with dust build up

Observed exhaust fan in room 109 restroom not working. CA: Management will have replaced

Observed sheets/bedding with staining on both beds in room in rooms 215 and 317. Discussed with PIC to replace sheets in room

Observed ice machine on first floor with pink residue buildup and holding water in bottom drain tray area (third floor machine not in operation at time of inspection). Discussed with PIC to have cleaned and maintain a frequency of cleaning in order to prevent future build-up, and to have drain looked at and resolved

Observed inspection from 01/06/2021 scored 99 posted behind front desk. Last inspection was completed 06/12/2023, which should have been posted. Will provide printed inspection today in field

Observed portions of both ice machines (first and third floor) that come into direct contact with ice with visible black/pink build up and slime

Observed smoke detector in room 202 not working. CA: Discussed with PIC. Batteries will be replaced or smoke detector will be replaced before room is rented out

Observed trash (cigarette butts, qtips, drink lids, etc.) and debris in stairwell. CA: Discussed with PIC. Hallways and stairwells need to be properly maintained. PIC will ensure these areas are cleaned and properly maintained moving forward

Observed hairs on the toilet and bathroom countertop in guest room 113. CA: Discussed with PIC. Room will be re-cleaned before it is rented out

Observed significant/strong odor in guest room 113. CA: Discussed with PIC. Room will not be rented out until issue is resolved
