Basic - OBSERVED: OPERATOR DOES NOT PERMIT CUSTOMERS TO USE BATHROOM. AT TIME OF INSPECTION OBSERVED BATHROOMS BLOCKED WITH TABLE AND CHAIRS WITH NO ACCESS TO BATHROOM BY GUESTS. EMPLOYEE REMOVED TABLE AND CHAIRS AND TOOK OUT OF ORDER SIGN OFF MEN'S BATHROOM DOOR. EMPLOYEE REMOVED MOP BUCKET AND MOP WHICH WAS STORED IN THE HALLWAY IN FRONT OF MEN'S BATHROOM.

OBSERVED: COVERED WASTE RECEPTACLE NOT PROVIDED IN WOMEN'S BATHROOM.

High Priority - Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed two employees changing tasks applied gloves no hand wash. Educated both employees on correct hand washing procedures. Corrective Action Taken

High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed 1 small flying insect in kitchen area near pastries, did not observe small flying insect landing on food contact surfaces.
