Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Ice buildup on all 3 reach in freezer at front; Fry, sausage and burger Reach in freezers Also the door to the walk in freezer has ice buildup at bottom.

Basic - No handwashing sign provided at a hand sink used by food employees. Missing at drive through sink. Emailed GM hand washing sign.

Basic - Wall soiled with accumulated grease, food debris, and/or dust. Wall behind 3 compartment sink has black mold like substance on it.

Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed ceiling tiles on cook line soiled with grease and dust.

Basic - Floor soiled/has accumulation of debris. Observed floor under kitchen equipment and in dry storage soiled with food debris and water.

Intermediate - Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. Observed no written procedures on-site and no on-site bleach available for DBPR handout. Discussed with person in charge.

Basic - Accumulation of dead or trapped birds, insects, rodents, or other pests, in control devices. Observed one glue trap with approximately 4 roaches in dry storage area. Person in charge had personal remove glue trap from store. Corrected On-Site

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed splash area behind soda fountain nozzles soiled with food debris.

High Priority - Employee touched soiled apron/clothes and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed employee wipe debris from shirt while wearing single service gloves then proceed to pour and attach lid on a slushee for drive through customer without changing gloves or washing hands.

Intermediate - Commercially processed ready-to-eat, time/temperature control for safety food opened and held more than 24 hours not properly date marked after opening. In the reach in cooler next to the ice cream machine, open container of cream stored from the previous day with no product date marking.

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Soda dispenser in the lobby area has mold like substance build up around the ice chute areas. Soda dispenser nozzles at drive through window has mold like substance at the top of the nozzles.

Intermediate - Handwash sink used for purposes other than handwashing. Hand wash sink at the front drive through window used to store tongs. Operator removed the tongs from the hand wash sink. Corrected On-Site

Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Ceiling vent over the main cook line has accumulation of dust.

Basic - Employee with no beard guard/restraint while engaging in food preparation. One employee actively working with food on the main cook line with no beard guard.

Basic - Ice bucket/shovel stored on floor between uses. Ice buckets stored on the floor in front of the bulk ice bin. Operator relocated the ice bins to the correct storage location. Corrected On-Site

Basic - Plumbing system in disrepair. Hand wash sink at the front service counter in disrepair, covered with plastic trash bag. Operator has work order in for the hand wash sink. Corrective Action Taken

Basic - Single-service articles improperly stored. In the dry storage/employee break room, container of single service drink lids stored on the ground.

Basic - Standing water in bottom of reach-in-cooler. Reach in cooler next to the ice cream machine has standing water at the base of the cooler.

Basic - Wet wiping cloth not stored in sanitizing solution between uses. On the main cook line, wet wiping cloth stored on the counter in front of the hot holding unit. Employee relocated the wet wiping cloth. Corrected On-Site
