Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels at handwashing sink next to office. Observed employee refilling paper towels. Corrected On-Site

Basic - Equipment in poor repair. Observed torn gaskets at reach in coolers at pancake freezer.

Basic - Interior of oven/microwave has accumulation of black substance/grease/food debris. Observed microwave with ovens with food splatter on fan guards.

High Priority - Employee touched soiled apron/clothes and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed manager tucking in shirt, then go back to bagging and traying food with out washing hands first.

Intermediate - Handwash sink not accessible for employee use at all times. Observed at front counter to the left of drive thru, the hand sink blocked with a trash can. Observed General manager correcting on site by moving trash can to a different location. Corrected On-Site

Basic - Equipment and utensils not properly air-dried - wet nesting. Observed wet nesting of cambros in dish area. District Manager separated cambros to properly dry. Corrected On-Site

High Priority - Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed employee use wipe cloth while not wearing gloves. Employee then put on gloves without washing hands and return to prepping ready to eat food.
