Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Upon inspection observed multiple air conditioning vents soiled inside the establishment. Also observed multiple vents to have buildup of condensation.

Basic - Employee personal food not properly identified and segregated from food to be served to the public. Upon inspection observed bag of employee drink in reach-in freezer. Also observed can of energy drink in walk-in cooler.

Basic - Equipment and utensils not properly air-dried - wet nesting. Upon inspection observed plastic storage containers wet nesting on dish rack in storage area of establishment.

High Priority - Toxic substance/chemical improperly stored. Spray bottle of stainless steel polish on shelf above triple sink. Operator removed bottles and placed on chemical shelf. Corrected On-Site

High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Vacuum breaker not attached to splitter on mop sink.
