Intermediate - Incorrect chemical test kit provided for measuring the concentration of the sanitizer solution used in the three-compartment sink and/or dishmachine. No chlorine test kit available. Only quaternary, per operator uses bleach.

Basic - No conspicuously located ambient air temperature thermometer in holding unit. No ambient air thermometer in standup reach in cooler.

Basic - Single-service articles improperly stored. Single service portion cups stored on the floor, operator relocated. Corrected On-Site

Basic - Mobile food dispensing vehicle license number not permanently affixed on the side of the unit in figures at least 2 inches high and in contrasting colors from the background of the vehicle. At time of inspection no license number posted.

Basic - Reach-in cooler interior/shelves have accumulation of soil residues. Interior of reach in freezer its frozen spilled drink residue.

Basic - Working containers of food removed from original container not identified by common name. Brown sugar in bulk container with no label.
