Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils. On top of shelf above the prep table. manager moved Corrected On-Site

Basic - Garbage on the ground and/or pad around dumpster. Boxes and old chairs around dumpster.

Basic - Working containers of food removed from original container not identified by common name. Water container not label. Container of hot chocolate on side. manager labeled Corrected On-Site

Basic - Worn, torn and/or soiled floors/carpeting. With food debris in walkin freezer floor.

Basic - Accumulation of black/green mold-like substance in the interior of the ice machine. Left side of the blue lining in the interior of the ice machine.

Basic - Bowl or other container with no handle used to dispense food. Employee used a cup to scoop ice at the beverage station near the hand washing sink.

High Priority - Displayed food not properly protected from contamination. Donuts on cart on frontline exposed to customers. Rack of donuts was moved to back kitchen Corrected On-Site

Basic - Dumpster rusted out on bottom. Operator called city for replacement. Corrective Action Taken

Basic - Wet wiping cloth not stored in sanitizing solution between uses. Wet wiping towels stored on counter. Operator had towels moved. Corrected On-Site

High Priority - Quaternary ammonium sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. 400 ppm. The manager remade the quaternary solution and called the servicer. Corrective Action Taken

Basic - Coffee filters not stored in a protected manner to prevent contamination. Observed on top of coffee machine

Basic - Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observed employee preparing drink with jewelry on that's not a wedding band. Operator removed when prompted. Corrected On-Site

Basic - Food stored on floor. Observed boxed soda and juice containers on floor in walk in cooler.

Basic - Open dumpster lid.

Basic - Intermediate - Accumulation of black/green mold-like substance in the interior of the ice machine.
