High Priority - Non-food grade paper/paper towel used on top of lettuce at salad station. Discussed use of food grade materials with operator.

Intermediate - Manager or person in charge lacking proof of food manager certification, (expired two days ago).Certification test taken and awaiting results. Other manager has current food protection manager certification.

Basic - Open dumpster lid, shared dumpster.

Basic - Plumbing system in disrepair, handle on hand wash sink not working, operator remove and is operating handle with locking pliers until he gets the part to fix it permanently. Handle broke while inspector was at location. Corrective Action Taken

High Priority - Operating with an expired Division of Hotels and Restaurants license.

Basic - Accumulation of scale build up in the interior of the ice machine/bin. Scale build up in ice bin.

Basic - In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Scoop handles for dry salad toppings in bowls, operator moved them.
