High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Observed no vacuum breaker installed on splitter attachment at mop sink.

Basic - Plumbing system in disrepair. Observed hot water handle at kitchen hand wash, next to flip top cooler, sticks in on position after use.

Basic - Storage of maintenance equipment/tools in areas that may result in cross contamination of food, equipment, utensils, linens and/or single-service and single-use articles. At back dry storage area- vacuum stored over single service containers- operator removed.

Intermediate - Handwash sink not accessible for employee use at all times. At area by triple sink- handwash sink blocked by trash can- operator removed. Corrected On-Site

Intermediate - No currently certified food service manager on duty with four or more employees engaged in food preparation/handling.
