High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Splitter attached to mop sink, no vacuum breaker at either end. Showed operator an example of vacuum breaker and they took picture.

Basic - Cutting board has cut marks and is no longer cleanable. Cutting board on maketable at end of cook line.

Basic - Food storage container/container lid cracked or broken. Lid for rice bulk container cracked, lid on tomatoes at front counter cracked, and lid at bulk containers towards office had duct tape.
