Observed 38 dead roaches found in the following locations: - 1 dead on floor by seven boxes of cooking oil, - 1 dead on floor in back storage room between crates with panko and other food on them, - 6 dead inside the unused ice machine compressor area, - 2 dead on floor under unused ice machine, -1 dead on floor along wall behind table with equipment on it in the back storage room, - 6 dead on floor around 23 boxes of used cooking oil, - 2 dead on floor along wall with the Certifed Food Manager certificates and business licenses, - 1 dead on floor behind chest freezer in the kitchen, - 10 dead on floor under table to left of grill, - 1 dead on the windowsill next to the make line, - 1 dead on floor behind make line, - 1 dead on floor to right of employee entrance to kitchen, - 2 dead on floor to left of beverage refrigerator in lobby area, - 2 dead on floor to left of bathroom door, - 1 dead on floor to right of door to back storeroom near the bathroom. Basic - Exterior door has a gap at the threshold that opens to the outside. 1. Front door into lobby has a gap. 2. Back door into storage room has a gap.

High Priority - Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature: - reach-in cooler in kitchen had ziploc bags of chicken in the same container as a ziploc bag of pork. Employee removed the pork and put in another container for proper food storage. Corrected On-Site

High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb: - two nozzles in back room do not have backflow preventers on them.
