Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed soiled ceiling vents in women's restroom.

Basic - Floor tiles missing and/or in disrepair. Observed ceiling tile not placed behind the beverage machine by front counter.

Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Observed ice buildup up inside walk in freezer.

Basic - Interior of oven/microwave has accumulation of black substance/grease/food debris. Observed soiled interior of microwave by ice machine.

Basic - No handwashing sign provided at a hand sink used by food employees. Observed in the men's restroom. The manager put up a sign. Corrected On-Site

Intermediate - Handwash sink not accessible for employee use at all times. Observed cart with trays blocking hand was sink next to 3 compartment sink. Employee moved cart away from hand was sink. Corrected On-Site
